Need a work or school note? Members can request a Verification of Treatment (VOT) through an E-Visit on kp.org.
Instead of making a call, you can log into your kp.org account and request a Verification of Treatment (VOT) quickly and easily using an E-visit.
What is a VOT E-Visit?
This E-visit can be used to request a work or school note due to a medical need. Notes are issued at the discretion of the E-visit physician care team based on medical appropriateness and are not guaranteed.
How to request a VOT through an E-Visit:
- Go to kp.org and select Get Care Now.
- Select “Start E-visit” from the menu
- Choose the “Work or School Note” option and answer a few health-related questions.
- A Kaiser Permanente clinician will review your request and medical chart.
- If approved, your note will be available in your After Visit Summary under “Visits” in your medical record.
Important information about VOT E-Visits:
- Notes provided through an E-visit allow for up to 3 days of absence.
- Notes are issued based on medical appropriateness at the clinician’s discretion.
- VOTs cannot be issued for past dates.
- You may submit a VOT request once every 8 weeks through an E-visit.
- You typically won’t receive a phone call after completing an E-visit.
